WashU IT OCM revises ServiceNow request form to enhance the requester experience

The WashU IT Organizational Change Management service request form has been revised to enhance the customer experience. The new OCM Service Request form is now available in ServiceNow. 

Revising the form is one of several changes the IT OCM Team implemented during FY25 to better align with Prosci Change Management methodology. The new form streamlines the requester experience. It clarifies familiar questions, simplifies others, and has been designed to align more closely with OCM Intake processes and procedures.  

The Team engaged a focus group of frequent requesters from across WashU IT to assist in developing the revised form. Other changes implemented in FY25 included revising the questions asked during OCM Intake meetings to complete the OCM Intake Form and creating a companion slide deck to guide meeting participants in answering the questions. 

WashU IT OCM will continue embedding change management methodology in IT processes and procedures throughout FY26, including the addition of future training and reference resources.
